Transition Coordinator – Anticipated – for LIFT program at Pembroke Academy

Pembroke Academy is seeking a certified special educator to coordinate the LIFT (Learning Independence for Transition) program for students with disabilities, primarily for the ages of 18 to 21.

The right candidate will be motivated, resourceful and able to work independently to assist students with developing employment and soft skills and independent living skills.  The Coordinator will be responsible for establishing internships and other work based experiences such as job shadows, business tours, volunteering and exploration of leisure activities.  The Coordinator will also need to work closely with outside agencies to assist the student and families with transitioning to adult services upon completion of the program.  Organization of student schedules, assigning para supports and transportation needs a must, with the assistance of the Exceptional Services Department.  Solid knowledge of the special education process and Indicator 13, creation and implementation of IEPs is required.
